© 2020 Elsevier España, S.L.U.Introduction: Different tools have been developed to assess the autonomy of general surgery residents, such as the System for Improving and Measuring Procedural Learning (SIMPL) scale. This tool consists of a 3-question assessment for the resident and for the surgeon, where the performance of residents in surgical procedures of various complexities is evaluated. The objective of this study was to analyze the autonomy of general surgery residents using the SIMPL scale. Methods: Based on the SIMPL scale, the autonomy of a group of residents belonging to the general surgery program of Tecnologico de Monterrey was evaluated in a period of 6 months. Results: A total of 124 evaluations were made; 41.12% (n = 51) were junior residents, and 58.8% (n = 73) were senior residents. A higher tendency was observed in junior residents, to require demonstration and explanation as a form of guidance during surgery (n = 22, 39.22%, P = 0.0093, OR = 2.977), likewise, higher rates of junior residents were observed to required active help (n = 13, 25.49%; r = 1.57) and passive help (n = 18, 35.29%; r = 1.35). All the residents who were only supervised were senior residents (n = 26, 35.62%; P < 0.001). Conclusion: Objective evaluations of the residents¿ performance were obtained. SIMPL is an effective tool to use, as it adapts and integrates the development of a specific procedure by measuring the autonomy of the resident, with the aim of seeing if the resident is ready to perform a procedure without supervision.
