All rights reserved.Purpose: The purpose of the study was to report on the management of a patient with primary corneal guttae symptoms after cataract surgery with trifocal intraocular lens (IOL) implantation. Case report: This is the case of a 67-year-old man referred to our unit due to persistent blurred vision and night glare 6 months after phacoemulsification and trifocal IOL implantation. Corneal guttae was observed on the slit-lamp examination, and changes to the morphology of corneal endothelium indicative of primary corneal guttae. A monofocal IOL exchange was performed to reduce corneal aberrations with excellent outcomes at 2-year follow-up. Conclusions: Corneal guttae accent optical aberrations in combination with premium IOLs can result in high visual disatisfaction. Screening for endothelial dysfunction is a key element to have successful outcomes in cataract surgery.
